BPH has a high recurrence rate.

Benign prostatic hyperplasia (BPH) is a male condition in which the prostate enlarges abnormally and compresses the urethra, causing voiding dysfunction.
It occurs mainly in men in their 50s and older, and hesitancy, a sensation of incomplete emptying and nocturia are its typical symptoms.
Many people dismiss it as a sign of aging and leave it untreated, but it can be managed well with early diagnosis and treatment.

BPH: how common is it?

BPH affects more than half of men in their 50s and rises sharply from the 60s onward.
It is a common condition among middle-aged and older men, affecting about 70% of men in their 70s and up to 80% of those aged 80 and above.
Because the prostate grows with age, it is best to stay ahead of it with regular checkups.

BPH: why does it occur?

BPH cannot be explained by aging alone.
It develops through a combination of factors, including changes in male hormones, cellular aging, family history, lifestyle and chronic inflammation.
Lifestyle habits such as lack of exercise, heavy drinking and obesity in particular raise the risk, so early management is needed.
  • Male Hormones

    Male Hormone Changes

With age, the balance between male and female hormones
is disrupted, causing the prostate to enlarge.
  • Facial changes in men due to aging

    Aging

As men age, prostate cells proliferate
and functional changes occur, leading to enlargement.
  • DNA

    Genetic Factors

If a family member has BPH,
genetic factors increase the risk of developing it.
  • Alcohol & Tobacco

    Lifestyle

Lifestyle habits such as inactivity, heavy drinking, and obesity
accelerate the progression of prostate enlargement.
  • Inflammatory cells

    Chronic Inflammation

Micro-inflammation inside the prostate causes tissue changes
and proliferation, contributing to prostate enlargement.

BPH:
what symptoms accompany it?

BPH causes no particular symptoms at first, but voiding dysfunction develops as the prostate enlarges.
Typical symptoms include a weak urine stream, urinary frequency, a sensation of incomplete emptying and nocturia, and usually 2 to 3 of them appear together.
Left untreated, these symptoms can greatly reduce quality of life, so early treatment is needed.
  • Weak Stream

The urine stream weakens and gradually thins.
  • Frequency

The number of times you urinate increases.
  • Residual Urine Sensation

You feel as if urine remains even after voiding.

  • Urgency

Severe urgency: urine cannot be held and small amounts leak.
  • Nocturia

You wake at least once during sleep to urinate.

Could my symptoms be BPH, too?

Don't leave even mild symptoms unattended.
You can check your health with a 1-minute self-assessment.

BPH diagnosis:
early management is the first step in treatment.

BPH needs to be distinguished from symptoms that simply come with age.
Urinalysis, digital rectal examination, ultrasound examination and other tests can accurately assess the size and function of the prostate.
Because an accurate diagnosis determines the treatment direction and prognosis, early screening is more important than anything.
Specialist reviewing the questionnaire

Medical History & Questionnaire

Key symptoms are checked with the international-standard (IPSS) questionnaire
to assess the degree of voiding discomfort and its impact on daily life.
Urinalysis

Urinalysis

Analyzes the urine for inflammation, hematuria, and other abnormalities,
and is used not only for prostate disease but also to distinguish it from bladder disease.
Performing uroflowmetry and residual-urine testing

Uroflowmetry & Residual Urine Test

Measures the speed of the urine stream and the amount of urine left after voiding
to check how much the prostate is blocking the urethra.
Digital rectal exam illustration

Digital Rectal Examination (DRE)

A palpation exam through the anus checks the size of the prostate and whether nodules are present,
helping distinguish BPH from prostate cancer.
Transrectal prostate ultrasound monitor screen

Transrectal Prostate Ultrasound

Ultrasound checks the size and shape of the prostate and whether stones are present,
and the findings are reflected in the treatment plan.

Urodynamics / Urocuff Test

Comprehensively analyzes voiding function, which is useful for distinguishing
simple BPH from a nerve or bladder problem.

BPH treatment:
how is it carried out?

BPH treatment should take a different direction depending on the symptoms.
Mild cases can be managed with medical (drug) therapy, but if symptoms recur or seriously disrupt daily life, a procedure or surgery may be needed.
Based on an accurate diagnosis, Goldman provides the treatment best suited to each patient.

Resolve your questions about BPH right now.

Goldman's know-how gives clear answers to patients' questions.
  • In BPH treatment, it is important not simply to reduce the size of the prostate but to decide on the treatment method while also considering the location of the enlarged tissue, bladder function, and voiding patterns. Even with the same BPH, the cause and the pattern of obstruction differ from patient to patient, so a precise diagnosis and a customized treatment strategy can affect the outcome.

Considering these characteristics, Goldman Urology operates a stepwise BPH treatment system spanning precise diagnosis, surgical-method selection, and post-treatment care.

First, precise diagnosis of prostate size and location and any bladder dysfunction lets us analyze each patient's cause of obstruction and set the scope of treatment.Combining PSA testing, prostate ultrasound, uroflowmetry, and urodynamic studies, we diagnose by function, not just by symptoms.

Then, depending on the patient's prostate size and condition, we choose among HoLEP, Aquablation, Rezum, UroLift, iTind, and other methods and apply the right surgery or procedure. Our hallmark is a strategy built around the patient's condition, not one fixed method.

After treatment, we keep tracking voiding changes with uroflowmetry and residual-urine checks, and through a revision-surgery clinic and follow-up based on long-term clinical data, we manage both recovery and the risk of recurrence. In this way, systematically designing the whole process — cause analysis, treatment choice, and aftercare — is the hallmark of Goldman's BPH treatment.

  • When choosing a hospital for BPH treatment, rather than a place that applies only medication or one specific surgery, it is important to check whether it has a stepwise care system spanning accurate cause assessment, treatment selection suited to the patient's condition, and post-treatment care. This is because BPH treatment can vary depending not only on prostate size but also on location, bladder function, and voiding patterns.

Therefore, it helps to also review the following criteria.

    - Whether a urology specialist provides care and a dedicated BPH clinic is available - Whether there is experience with medication, procedures, and surgery across various prostate sizes and symptom stages - Whether precise evaluations such as prostate ultrasound, PSA testing, uroflowmetry, and urodynamic studies are available - Whether systematic follow-up checks voiding improvement and recurrence after treatment

Because the right BPH treatment can differ by bladder function or obstruction location even for prostates of the same size, it is important to pinpoint the cause through precise diagnosis and set an individualized strategy before treatment.

For example, at Goldman Urology the treatment direction is set through a precise diagnostic process that checks prostate size and location and any bladder dysfunction, and among various treatment methods such as HoLEP, Waterjet (Aquablation), Rezum, UroLift, and iTind, a plan suited to the patient's condition is applied step by step.

After surgery, voiding changes are monitored with uroflowmetry and residual-urine checks, and a management plan that accounts for possible recurrence helps patients expect more stable outcomes.

  • In BPH the whole prostate enlarges, but the condition specifically means abnormal tissue forming like a lump around the urethra and causing voiding difficulty.BPH surgery removes exactly this unnecessary lump.So the outcome depends on how accurately and cleanly the enlarged tissue is removed.
  • The outcome of BPH surgery depends on when and how it is performed. At Goldman Urology, we focus on the following.

    1. Symptom severity and quality of life: We assess how much nocturia, residual urine, and retention disrupt daily life despite medication, and whether surgery is now needed.

    2. Prostate size/shape and comorbidities: We choose the surgical method (HoLEP, Aquablation, etc.) after reviewing prostate size/shape, bladder function, infection/stones, and any heart disease, diabetes, or anticoagulant use.

    3. Understanding the changes after surgery: We explain in advance not only the degree of voiding improvement but also possible sexual-function changes such as retrograde ejaculation, and together decide a treatment direction that fits the patient's occupation and lifestyle.

    4. Surgeon experience, equipment, and recovery: We review surgical experience, the laser equipment used, the hospitalization/recovery period, and the recurrence/complication management plan so patients can choose with peace of mind.

  • Whether and when it recurs depends on how completely the abnormal prostate tissue that causes BPH is removed.

    · UroLift (prostatic urethral lift): The enlarged tissue is held aside with implants, not removed, so recurrence is possible.

    · TURP, Plasma TURP: The immediate result is good, but due to enlarged tissue that is not completely removed, the recurrence rate is known to be about 15% at 5–10 years after surgery.

    · HoLEP: Because the enlarged tissue is completely removed, recurrence is very unlikely.

  • Which BPH surgery is appropriate depends on the prostate's size, location, and shape, the patient's symptoms, and any complications!Rather than picking a surgery someone else said was good, choose the method that suits you.Decide what fits you after a full consultation at a place where every surgery is possible.Even surgeries with the same name do not remove the same amount of enlarged prostate. At Goldman, every surgery is possible, and each has a customized system led by experienced medical staff, so the optimal surgery can be performed for each individual.
